Form 1098-T Tuition Statement

The Form 1098-T Tuition Statement reports the amount of tuition and related fees that the student has paid Fox Valley Technical College for qualified tuition and related expenses during the calendar year. The purpose of the form is to assist the IRS and you in determining if you are eligible for any tax incentives, including the American Opportunity Credit or the Lifetime Learning Credit.

What is the IRS Form 1098-T?

The Form 1098-T Tuition Statement reports the amount of tuition and related fees that the student has paid Fox Valley Technical College for qualified tuition and related expenses during the calendar year. The purpose of the form is assist the IRS and you in determining if you are eligible for any tax incentives including the American Opportunity Credit or the Lifetime Learning Credit.

In compliance with IRS guidelines, the Form 1098-T the student receives is information that was sent to the IRS by Fox Valley Technical College.   

There is no IRS requirement that you must claim the tuition and fees deduction or an education tax credit. Claiming education tax benefits is a voluntary decision for those who may qualify.

 

Does FVTC have my current mailing address, phone number(s) and preferred email on file?

Sign in to your MyFVTC student account. On the right side of the page, make sure that your “mail” address is accurate, along with your phone number(s) and preferred email. This information is important in case we ever need to get in contact with you.

Why does FVTC need my Social Security Number? What is the W-9S form?

FVTC is required by the Internal Revenue Service (IRS) to send Form W-9S, Request for Student’s or Borrower’s Taxpayer Identification Number for purposes of 1098-T Tuition Statement reporting.

Form W-9S is sent to eligible students who were enrolled in credit based courses during the tax year and who have missing or invalid Social Security Number in our database.

By the end of December, students should complete the form and return it to the Student Financial Services office (room E118) on the Appleton Campus or mail it to FVTC, Attn: Student Finance, 1825 N Bluemound Drive, Appleton, WI 54914. If the form is not returned, FVTC will report the student’s Form 1098-T Tuition Statement to the IRS without a taxpayer identification number.

When and where will the Form 1098-T be available?

Form 1098-T will be available by January 31 for eligible students. Students can view and print their Form 1098-T through eFAS (Financial Account Suite). An email notification will be sent to the student’s preferred email when the Form 1098-T is available. To sign into eFAS, students need their 9 digit student ID number and their MyFVTC password. If students can’t remember their password, please click the “reset password” link on the MyFVTC page to create a new password.

If the student has not chosen to “opt in” to the electronic version of the Form 1098-T in eFAS, students will be mailed a paper copy postmarked by January 31.

What information is on the Form 1098-T?

Box 1 – Actual payments received for qualified tuition and related expenses in the calendar year shown on form. This amount does NOT include books.
Box 2 – FVTC does not use this box
Box 3 – Not checked. FVTC has not changed its reporting method
Box 4 – Adjustments made to a prior year’s payment received for qualified tuition and related expenses. This may change any allowable education credit you may have claimed for a prior year.
Box 5 – Scholarships or grants received in the calendar year shown on the form.
Box 6 – Adjustments made to a prior year’s scholarships or grants received. This may change any allowable education credit you may have claimed for a prior year.
Box 7 – This box will be checked if the amounts in box 1 include amounts for an academic period beginning January – March of the following calendar year.
Box 8 – This box will be checked if you are enrolled half-time or greater.
Service Provider/Acct No – This will be your 9 digit FVTC student ID number.

What amounts are included in Box 1?

Qualified tuition and related expenses are defined as tuition and certain related expenses required for enrollment or attendance at an eligible educational institution. This would include any credit classes that can apply towards a degree or certificate.

Box 1 reflects the actual amount paid (for qualified tuition & expenses) on the student account within the calendar year that just ended. Many students enroll and pay for spring terms in November and December of the previous year.   

Examples – 2021 spring classes paid in Nov/Dec 2020 would have received the 2020 1098-T. 2022 spring classes paid in Nov/Dec 2021 will have those amounts showing on the 2021 Form 1098-T.

Unqualified expenses include insurance, application fee, locker fee, late fees, books and supplies purchased at the Campus Store. You need to keep your receipts for books purchased as FVTC does not have access to those amounts.

Why are my spring classes included in the amounts paid for the calendar year?

The Form 1098-T box 1 reflects the actual amount paid (for qualified tuition & fees) on the student account within the calendar year that just ended. Many students enroll and pay for spring terms in November and December of the previous year. 

Examples – 2022 spring classes paid in Nov/Dec 2021 would have received the 2021 1098-T. 2022 spring classes paid in Nov/Dec 2022 will have those amounts showing on the 2022 Form 1098-T.

Why can’t I view my Form 1098-T with my mobile device?

Please use a desktop or laptop computer and a non-app version of your browser. 

The Form 1098-T is in PDF format. You must have Adobe Acrobat Reader 8.x or higher to view your 1098-T Tuition Statement. It will not open with an App version of your browser.

What if the social security number is incorrect on the Form 1098-T?

If FVTC does not have your correct social security number on your student account, the Form 1098-T will be sent to the IRS without a valid social security number. This will make it difficult for the IRS to verify your tuition statement with your tax returns

Please check the last 4 digits of the social security number on the form. If they are not correct, please fill out Form W-9S (Request for Student’s or Borrower’s Taxpayer Identification Number) and send it to FVTC prior to March 15. We will correct your student record and re-send you a Form 1098-T with the correct social security number.

Who is NOT eligible to receive a Form 1098-T?

In compliance with IRS guidelines – FVTC is not required to file Form 1098-T or furnish a statement for:

  1. Students whose qualified tuition and related expenses are covered by a formal billing arrangement between FVTC and the student’s employer or a government entity. This includes Apprenticeship students who pay through payroll deduction, directly to their employer, or a third party that is not FVTC, ie. ABC of Wisconsin Apprenticeship.

  2. Students who took courses for which no academic credit is offered or credits are not applicable to the degree being sought. Examples – hobby classes, motorcycle classes, driving classes, various tests.

  3. Students who paid for the spring term in the prior calendar year - these tuition charges would have appeared on your previous year’s Form 1098-T. FVTC will only provide a Form1098-T for actual payments received in the calendar year.

How can I get a duplicate copy of Form 1098-T? Can you fax or email it to me?

For security of your information, we will not fax or email Form 1098-T.

Even if you did not “opt in” for the electronic version, you can view and print a copy your Form 1098-T through eFAS (Financial Account Suite). An email notification is sent to student’s preferred email when the Form 1098-T is made available, by January 31. To sign into eFAS, students need their 9 digit student ID number and their MyFVTC password. If students can’t remember their password, please click the “reset password” link on the MyFVTC page to create a new password. You can still access your account even if you are no longer an active student.

If students cannot remember their student ID number and password to get into MyFVTC or eFAS, the requested duplicate Form 1098-T will be mailed to the most recent mailing address on the student account.

If the social security number is incorrect, you will need to complete Form W-9S prior to March 15.

Am I eligible for an Educational Tax Credit?

There is no IRS requirement that you must claim the tuition and fees deduction or an education tax credit. Claiming education tax benefits is a voluntary decision for those who may qualify.
To determine the correct amount, if any, to enter on your tax return, please consult your tax advisor and your personal expense records. Please refer to IRS Publication 970.
