Congratulations on your decision to enhance your healthcare knowledge and skills. We are excited to welcome you to the upcoming program class(es). As part of your registration, there is an important requirement for maintaining registration and ensuring your successful participation in the class.
To comply with our clinical documentation standards and ensure a safe and effective learning environment for all students, it is mandatory for all participants to provide the required health documentation. This documentation is essential for your clinical placements and must be completed by the specified deadline. Failure to do so will result in your being removed from program classes. If you are removed from program courses, you will not be allowed to re-register until the documentation is complete and only if space is available and placement capability. Step 2: Complete & Upload Health Information
- You must upload all required health documentation to the Viewpoint platform by the documentation deadline. Documentation is not complete until it has been approved. This includes but is not limited to immunization records, health screenings, and any other pertinent health information.
- Please ensure that all documents are submitted in a timely manner to avoid any delays in system approval or your clinical placements.

- Schedule your American Heart Association BLS Provider CPR class (if your current certification will expire before the documentation period).

- Schedule appointments to get your TB test done.
You can get this done at your place of employment, your healthcare provider, or FVTC's Campus Care Clinic. FVTC offers free TB testing at our Campus Care Clinic. An appointment is required and can be scheduled by calling our provider, Prevea, at 833-625-0628.
- Check your required immunizations to make sure you have them.
Acceptable proof of vaccination is available from the Wisconsin Immunization Registry or your personal care provider (PCP). Documentation must have your identifying information on it, including name and date of birth.
